You will hear a telephone conversation between a patient and a receptionist at a dental clinic. Answer questions 1–8.

Receptionist: Good morning, Brightsmile Dental Clinic, Karen speaking. How can I help you?

Caller: Hi, good morning. I'd like to book an appointment to see a dentist, please.

Receptionist: Of course. Are you an existing patient with us, or is this your first visit?

Caller: It's my first time, actually. I've just moved to the area.

Receptionist: Welcome! I'll need to set up a new patient record for you. Can I take your full name, please?

Caller: Yes, it's Daniel Forsythe.

Receptionist: Could you spell the surname for me?

Caller: Sure, it's F-O-R-S-Y-T-H-E.

Receptionist: Thank you. And what's the reason for your visit, Mr Forsythe?

Caller: I've had a bit of toothache on the lower left side for about a week now. It's getting worse when I eat anything cold.

Receptionist: I see. That does sound uncomfortable. We can book you in for a check-up and the dentist will examine it. Now, what date would suit you?

Caller: Would it be possible to come in this week? Maybe Thursday?

Receptionist: Let me check. I'm afraid Thursday is fully booked. I could offer you Friday the fourteenth, in the morning.

Caller: Friday morning works for me. What times do you have?

Receptionist: I have nine fifteen or ten forty-five available.

Caller: I'll take the later one, please, at ten forty-five.

Receptionist: Perfect. The appointment will be with Dr Patel, one of our senior dentists. Now, the standard first consultation fee is forty-five pounds, but if any treatment is needed on the day, that would be charged separately.

Caller: That's fine. Should I bring anything with me?

Receptionist: Yes, please bring some photo identification and, if you have it, a list of any medications you're currently taking. Are you taking anything at the moment?

Caller: Just some tablets for high blood pressure.

Receptionist: Okay, I'll make a note of that. Could I also have a contact number for you?

Caller: Yes, it's oh-seven-seven-double-six, three-one-four, two-nine-oh.

Receptionist: Let me read that back: zero-seven-seven-six-six, three-one-four, two-nine-zero. Is that right?

Caller: That's correct.

Receptionist: And one last thing — how did you hear about our clinic?

Caller: A neighbour recommended you, actually.

Receptionist: Lovely. So that's confirmed: Friday the fourteenth at ten forty-five with Dr Patel. We'll send you a reminder by text the day before. Please try to arrive about ten minutes early to complete the registration form.

Caller: Will do. Thank you very much for your help.

Receptionist: You're welcome, Mr Forsythe. See you Friday.
